Epiduroscopy
The Targeted Disc Decompression (TDD treatment) is intended for patients who suffer from a herniated disc, a bulging nucleus of an intervertebral disc in the spine. This can cause pain which can radiate to the legs. the TDD is a treatment under mild sedation, which reduces the pressure in the intervertebral disc by means of heating the bulge, thereby reducing the pain, or making it disappear completely.
From the spinal cord, nerves run to the rest of the body through the epidural space. As a result of surgery or inflammation, scar tissue can form in the epidural space. The scar tissue may cause irritation to the nerve roots and lead to pain in the back and/or legs. An epiduroscopy is a form keyhole surgery at the level of a space on the inside of the spine, known as an epidural space. The doctor can observe in detail what’s wrong with the nerve roots and the surrounding tissue. Apart from the diagnosis, the doctor may also perform a treatment by injecting medication around the irritated nerve root(s), and optionally mobilize present scar tissue which can lead to extra space for the nerve root, and alleviation of the pain symptoms. Indications An epiduroscopy procedure can be performed for the following indications: Diagnosis and treatment of spinal pain syndromes Direct drug application Direct lysis of scarring caused by different kinds of surgeries (ie. hernia operation) or inflammation (ie. failed back surgery syndrome) Placing catheter system
The procedure is done in an operating theatre under sedation. After a local anesthetic at the level of the tailbone, a scope is inserted through the coccyx with the help of fluoroscopy. The catheter is steerable and once in the correct area the adhesions can be broken down by the catheter. During the procedure, a sterile fluid (0.9% NaCl) is injected into the epidural space to make room for the camera. With the help of the catheter, the doctor can, apart from making the diagnosis, try to clear out any possible adhesive clumps that might be present. These may cause as the catheter passes through the irritated area. Benefits A benefit of the epiduroscopy is that it can be used both diagnostically as well as therapeutically. Since the procedure is minimally invasive, there is no operation, which means there is a lower risk of complications. Results of an epiduroscopy used to treat pain complaints are typically noticeable within a few weeks.
