SI-Joint Injection
The sacro-iliac joints (SI joints), also called sacral joints, form the link between the sacrum and pelvis and thus between the back and legs. SI joints can be overloaded or inflamed. The pain is located in the lower back and can radiate into the buttock and thigh. An injection into the sacroiliac joint, with local anesthetic and anti-inflammatory drugs, can have a positive and analgesic effect.

The joints are located at the bottom of the back, on both sides of the spine. The joints can move very little, so the back and pelvis are quite sturdily connected together. SI joints can be overloaded or inflamed. The pain is located in the lower back, usually on one side, and radiates into the buttock. Sometimes the pain radiates into the thigh, but usually not past the knee. There may also be pain in the groin. Walking can be difficult and cause pain. The treatment takes place at the level of the sacrum (SI joint).
Preparing for the treatment The treatment takes place in the OCP Medical Center. The treatment is performed by an anesthesiologist from OCP Medical Center. You do not need to make any preparations at home unless specifically stated. You do not have to fast on the day of the treatment. You’ll arrive at the clinic about fifteen minutes before your treatment. An employee of OCP Medical Center will guide you to a waiting area. Before the procedure starts a nurse and / or doctor will bring you to the adjacent treatment room. The treatment During the SI block you lie on your stomach on the treatment table. A needle is inserted into the joint space of the sacrum with the help of fluoroscopy. The correct position is confirmed with the help of contrast fluid. When the needle is positioned in the right place, the medication (3 ml Bupivacaine 0.5% + 80 mgr. Depomedrol) is injected. The needle is removed and a bandage is placed on the spot of the injection.
After the treatment One usually hardly experiences side effects of the treatment and you may go home 15 minutes after treatment. When the local anesthetic wears off (about 1 to several hours after treatment) post-operative pain may occur. This is because a needle is inserted into an already irritated area. The pain may increase over the first few days, but usually doesn’t last longer than several days. You can take painkillers against the pain. This can be your own pain medication or paracetamol according to the dosage on the package leaflet. On the day of the treatment it is advisable to take it easy. You may do what feels okay, but hard work is strongly discouraged. As a precaution, it is not allowed to drive yourself home, so we advise you to ensure that someone can bring you home. Possible side effects Hematoma: this can occur if a blood vessel is damaged. This causes a bruise and sometimes afterpains. Muscle pain: at the spot of the injection. Temporary loss of strength: this can last for several hours and is caused by the anesthesia. Temporary reduction of sensation in the skin in the treated area: this is caused by the treatment and will return on it’s own after a few weeks. Based on the cause of your pain, your specialist will assess if a physical treatment is necessary. Based on the medical history the necessity of additional tests will be decided.
